- 1. Mail copies of transcript(s) (unofficial photocopies are acceptable) of all college/university coursework to:

- Credential Evaluations

- Hayward, CA 94542-3088
- 2. Attach a cover letter with your name, address and telephone number, along with any information you believe relevant to the interpretation and evaluation of your transcript(s), such as catalog descriptions of courses from other institutions.
- 3. Enclose a check for $50.00, payable to "CSUH", unless you are a CSUH graduate, a currently enrolled CSUH student, or an applicant for admission to CSUH. (Students in the last category should include a copy of their receipt for the application fee)

- Upon completion of the evaluation, you will receive information in the mail showing what coursework you have completed and which courses you still need to complete to meet the requirements of the Subject Matter Program and thus "waive" the CSET.
